Our client, a burgeoning health services organization, is seeking a dedicated Occupational Therapist. This role involves providing comprehensive occupational therapy services both at the Center and in-home settings, aiming to enhance participant independence and functionality. Collaborating closely with Primary Care Physicians, rehabilitation teams, and other interdisciplinary team (IDT) members, the Occupational Therapist plays a crucial role in developing and restoring participant functions to improve their quality of life.
Qualifications:
Bachelor’s degree in occupational therapy from an AOTA & ACOTE accredited program.
● Minimum of two years of occupational therapy experience in a clinical setting, preferably with a focus on the frail or elderly population.
● Preferred two years of supervisory experience with a strong track record in team development and mentorship.
● Certification by the National Board for Certification in Occupational Therapy.
● Licensed or eligible for licensure as required by the state of employment.
Essential Job Duties:
● Evaluate participants through Occupational Therapy assessments to determine care needs, preferences, and goals, actively contributing to IDT meetings for care plan development.
● Implement and document Occupational Therapy interventions as per physician orders and care plans, utilizing various techniques and devices to support muscle re-education, cognitive functions, and daily living activities.
● Oversee and train Therapy Assistants, ensuring effective delivery of Functional Maintenance Programs (FMP). ● Educate staff, participants, and their caregivers on Occupational Therapy practices relevant to their interactions with participants.
● Facilitate seamless care transitions across various settings and participate in comprehensive end-of-life care coordination.
